Using your Care Sequence® plan
Care Sequence® is your action plan during care.
It will help you be informed about what tests, treatments and services are in your plan and their approximate timing and order. Using your Care Sequence® will help you to actively participate in and be in control of your care.

A Care Sequence® plan provides an outline and approximate timeline of care you may get and describes the care indicated for you, in the order this care should occur. You may use different Care Sequences during your care.
Left column is your care checklist
Your care provider will discuss the checklist with you and will check items that you will need. Only the checked boxes represent your individual care plan. Ignore items that are not checked – they do not apply to you or are not needed for you.
Right column shows approximate timing and order of different care
- The time shows approximate month care may occur
- Squares are shorter events, such as consult or test.
- Longer bars stretch in time and may take several days or weeks.
- Arrows show steps that must be done prior to other steps. Keep this in mind when planning appointments.
Care Sequence layout also has three portions of care done in a specific order:
Top portion lists tests, procedures and appointments needed to evaluate your COPD and plan your treatment
These may take some time. Refer to the approximate time and order to know in what order these appointments will likely happen and how much time this may take.
Middle portion shows what happens when test results come back and the steps needed to start your treatment. These may include treatment selection and decisions, therapy education and other care.
Bottom portion shows you care that needs to continue. This portion lists important care that is beneficial to address your overall health needs and your support needs, such as practical, financial or insurance needs.
