Care Sequences® help people with COPD better understand their care plan and manage their care

 A Care Sequence® helps you and your healthcare team develop a personalized plan that fits your needs.  A Care Sequence® includes a care checklist, and a graphic chart of the timing and sequence of care.

There are different plans for different points in care.  

Choose the plan below that best fits where you are in your treatment. You may use different plans over time.
